On December 3, 2015, before me, Peter Jackson, Notary Public personally appeared John McIntosh, who proved to me on the basis of satisfactory evidence to be the person whose name is subscribed to the within instrument and acknowledged to me that he executed the same in his authorized capacity, and that by his signature on the instrument the person, or the entity //////upon behalf of which the person acted,////// executed the instrument.

From a form called "ALL-PURPOSE ACKNOWLEDGMENT" from a California notary public to certify that a document remitted to him by the person named John McIntosh contains 2 pages.
Said document is then sent to an overseas notary or lawyer.

disagree Daryo : "personne" is enough ["... physique" is an unnecessary addition]+ there is no need to replace "une entité" par "une personne morale" - especially given they are mostly but not exactly the same thing.
