H.C. is indeed historia clínica, medical records, but the actual record, where patient's data is recorded is called a "chart".

The difference is clear in the following text:

The chart begins upon admission of the patient to the hospital. A face sheet is created which contains all demographic information on the patient, i.e. name, address, date of birth, date of admission, insurance information, attending physician’s name, and any other information, which the hospital considers pertinent. This new chart with its new face sheet is then taken to the floor with the patient. Old chart/charts are retrieved from HIM and also sent to the floor to which the patient is admitted. ....
