I don't understand the "alternado" here? Does it mean alternating with the medication listed before or after the enantyum? It seems very cryptic to me.

Paracetamol 1gr cada 8 horas.
Enantyum 25mg cada 8 horas ***alternado tres días***.

Como médico debo aclarar que, aunque esta práctica está muy extendida, es controversial, y en general se recomienda elegir un sólo medicamento y no alternar.
Es correcto lo que dice @Chema, según la indicación las tomas serían, por ejemplo: paracetamol 08 hs, enantyum 12 hs, paracetamol 16 hs, enantyum 20 hs, etc.
Helena, Chema - in my note to the client I mentioned that I believe it means alternating paracetamol with enantyum with a 4-hour gap.

The next line is:

So the "intercalado" probably refers to paracetamol with enantyum, although I think I'll inform the client that it's far from clear.
