Yet another report, this time for a patient with an upper respiratory infection and mild facial cellulitis.

I don't understand the "alternado" here? Does it mean alternating with the medication listed before or after the enantyum? It seems very cryptic to me.

"Tratamiento:
Paracetamol 1gr cada 8 horas.
Enantyum 25mg cada 8 horas ***alternado tres días***.

Hola Mauro, aunque tangencial, me ha sorprendido tu comentario en relación a lo controvertido de combinar / alternar paracetamol y dexketoprofeno. Salvo en ámbito pediátrico, donde claramente se desaconseja la práctica no supervisada de intercalar pej. paracetamol+ibuprofeno para controlar la fiebre, en el tratamiento del dolor por ejemplo no era consciente de ningún tipo de controversia al respecto. ¿Tienes alguna referencia que pueda revisar? ¡Muchas gracias!

"Alternado" se aplica para el paracetamol y el desketoprofeno (enantyum). Ambos fármacos tienen efectos similares (antipiréticos, analgésicos), y hay médicos que los indican alternadamente en busca de mayor eficacia.

Como médico debo aclarar que, aunque esta práctica está muy extendida, es controversial, y en general se recomienda elegir un sólo medicamento y no alternar.

Es correcto lo que dice @Chema, según la indicación las tomas serían, por ejemplo: paracetamol 08 hs, enantyum 12 hs, paracetamol 16 hs, enantyum 20 hs, etc.

Helena, Chema - in my note to the client I mentioned that I believe it means alternating paracetamol with enantyum with a 4-hour gap.

One of my granddaughters is ill and yesterday my daughter explained the treatment to me, using similar words. I initially understood that she had to alternate antibiotics and paracetamol but no, she meant that she had to administer antibiotics at 08.00, 16.00, 00.00 and paracetamol at 12.00, 18.00, 02.00.

The next line is:
"Flumilexa cada 24 horas dos semanas." - sorry for mentioning it but without specifically stating it.

So the "intercalado" probably refers to paracetamol with enantyum, although I think I'll inform the client that it's far from clear.

Sí, una pauta habitual y razonable es recomendar tomar los AINEs (dexketoprofeno, ibuprofeno, etc) después de las comidas (tras desayuno, comida y cena), intercalando un analgésico, paracetamol por ejemplo (a media mañana, a media tarde y un tercer comprimido por la noche si hace falta, con tres tomas o tres gramos diarios máximo). Las formas de expresarlo en castellano son variadas pero trasladan básicamente la misma idea de "intercalar" las tomas.

I recently had severe pain with an ear infection and was prescribed paracetamol, ibuprofen and enantyum to be taken each day as prescribed, as the times between each dose of an individual one was too long. Only precaution is not to exceed the stated maximum daily dose of any of them

Creo que todos estamos de acuerdo con el significado: Tomar ambos medicamentos durante tres días, cada ocho horas cada uno, intercalando las tomas. Es lo habitual con paracetamol + un AINE. La palabra alternar cabría si se dijera ¨tomar cada 4 horas, alternando x y z¨por ejemplo.
Creo que estas son instrucciones para dar de alta al paciente no? Dado que se trata de un ambulatorio.

Efectivamente, tanto por el tipo de combinación (paracetamol + enantyum) como por el fraseo (cada 8 horas alternado) en este caso se refiere al anterior. Aunque en el fondo, la expresión original serviría de forma aislada;
"Administrar cada 8 horas", en un hospital por ejemplo, alude al horario de medicación regular. "Cada 8 horas alternado" indicaría en horario no regular; intercalado entre (alternando con respecto a) las tomas habituales.

@Chema Eso es lo que había entendido yo también, y sería lógico; pero como Neil se refirió al medicamento mencionado después del enanytum, me ha parecido que sería interesante saber cuál es. Si, por ejemplo, la siguiente línea empieza "con...", se podría plantear otra interpretación. Desde luego, no lo creo, porque paracetamol + enanytum + otro analgésico o antiinflamatorio sería raro.

Tomar paracetamol cada 8 horas
Tomar enantyum cada 8 horas, intercalado; intercalando entre las tomas de paracetamol; tomar entre toma y toma de paracetamol (durante 3 días).

Durante 3 días tomaré
Paracetamol a las 08 horas, 16 horas y 24 horas
Enantyum a las 12 horas, 20 horas y 04 horas

El texto original por sí sólo no es demasiado claro porque es un texto que seguramente complementa una indicación dada de forma oral. Se podría hacer aclaraciones como "leaving a gap of 4 hours between one and the other", pero mi recomendación es que no hace falta tal aclaración.

El siguiente es un fragmento de la enciclopedia virtual de medicina UpToDate sobre este tema, en la que se usa el término alternating:

"We do not suggest combining or alternating acetaminophen with ibuprofen because of the potential for dosing confusion, increased toxicity, and contribution to fever phobia. Although combining or alternating acetaminophen and ibuprofen may be more effective than either agent alone in reducing fever, it is not clear that this temperature reduction is clinically significant."
