auscultation findings The use of "souffle", which I suspect means "murmur" here, means that we are dealing with clinical notes following some auscultation (stethoscope) findings. The acronym dTT presumably relays some quality of the murmur (such as loud/soft/metallic, though not necessarily any of these) or perhaps its timing: de Temps Transient... ?
But I'm certainly not confident to post an answer without further research.

probably a typo for dIT - de IT - IT being insuffisance triscuspidienne.

The TR murmur is indeed systolic, so that fits.
