Impaired intellectual awareness is where a person is unable to understand that a deficit exists. The second type is impaired emergent awareness, where a person knows there is a problem but is unable to realise when the problem is occurring, or to compensate for the deficits.

First, patients with impaired self-awareness lack information about themselves. Second, they experience cognitive “perplexity” when they receive feedback about their behavioral limitations. Finally, they exhibit a cautious willingness or indifference when asked to consider new information about their level of functioning. In contrast, patients who deny their disability exhibit an implicit knowledge of impaired functioning. They actively resist information about their impairments and disabilities and exhibit some form of active (versus passive) response to the “new information” about themselves. Based on these broad criteria, an impaired self-awareness scale and a denial of disability scale were developed.

Anosognosia Rating Scale
The categorical three point scale from Reed et al.15 was used. This scale has high interrater reliability15 and corresponds well with other measures of anosognosia.3 Based on cognitive testing and interviews with the patient and the caregiver, a clinician rated the patient’s level of awareness. Awareness was classified into the following categories: “full awareness,” “shallow awareness”, and “no awareness.”


On the Anosognosia Rating Scale, 96 patients (30%) had “full insight,” 190 patients (59%) had “shallow insight”, and 35 patients (11%) were rated with “no insight.”
