I am translating Medical Reports sometimes and have come upon this term: Accident and Emergency Records (for the UK).
"road safety type medical records= fisa medicala tip siguranta circulatie" means a standard medical records form for road safety.

What I do not know is this: who issued this Certificate. If a GP did then I would translate : "road safety type medical records".
It may mean a periodical control of a professional driver's health for driving purposes.
