Source text - German Wirkungsweise : Morphin hemmt die Schmerzempfindung im Bereich des Thalamus und des Stammhirns, wodurch eine Schmerzregistrierung weiterhin möglich ist, der Schmerz aber nicht mehr als subjektiv störend empfunden wird.
Eingesetzt wird Morphin bei sehr starken Schmerzen, insbesondere bei Karzinomschmerzen und bei Schmerzen in den Körperhöhlen, z.B. postoperativ.
Morphine haben auch eine antitussive, d.h. hustenstillende Wirkung ; die Hemmung des Hustenreflexes läuft meist mit der analgetischen Wirkung parallel. Es gibt jedoch auch schwächer analgetisch wirkende Opioide wie Codein, die eine stark hustendämpfende Wirkung besitzen. Morphine beeinträchtigen auch die Temperaturregulation. Man kann daher mit Morphinen einen schweren Schüttelfrost durchbrechen.
Neben einer gesteigerten Schlafbereitschaft registriert man bei vielen Patienten eine Veränderung der Stimmung. Nicht immer tritt jedoch eine euphorische Stimmung auf; es kann auch zu einer dysphorischen Stimmung mit Angst- und Spannungszuständen kommen.
Verhalten des Medikaments im Körper: Morphin wird enteral gut resorbiert, unterliegt jedoch einem First-pass-Effekt in der Leber (Bioverfügbarkeit 15-30%); damit es nach oraler Applikation ausreichend wirkt, muss die orale Dosis gegenüber der intravenösen verdreifacht werden. Die maximale analgetische Wirkung ist bei intravenösen Applikation 20-30 Min. und bei intramuskulären Applikation 60-90 Min. nach der Injektion zu erwarten. Abgebaut wird das Morphin in der Leber; es wird nach Glukuronidierung über die Niere ausgeschieden. Abbau und Ausscheidung begrenzen die Wirkung des Morphins auf 2-3 Std.

La morphine est utilisée pour traiter des douleurs très intenses, notamment celles d’origine cancéreuse ou viscérale comme les douleurs postopératoires.
Les morphiniques ont aussi un effet béchique (c’est-à-dire antitussif) ; généralement, l’inhibition du réflexe de toux intervient parallèlement à l’effet analgésique. Cependant, il existe aussi des opiacés à action analgésique plus faible (comme la codéine) qui possèdent un effet antitussif puissant. Les morphiniques entravent aussi la régulation de la température. Par conséquent, ils peuvent être utilisés pour juguler un grand frisson.
Outre une envie de dormir accrue, on remarque chez beaucoup de patients un changement de l’humeur qui ne se manifeste pas toujours sous forme d’euphorie; on constate parfois de la dysphorie avec anxiété et tension.
Pharmacocinétique : La morphine est bien résorbée par le tube digestif mais subit un effet de premier passage hépatique (biodisponibilité : 15 à 30 %). Pour atteindre un effet suffisant lors de l’administration orale, il faut tripler la posologie utilisée par la voie intraveineuse. En administration intraveineuse, l’effet analgésique maximal est atteint 20 à 30 minutes après l’injection. En administration intramusculaire, il faut compter 60 à 90 minutes. La morphine est métabolisée par glucuronoconjugaison au niveau du foie et est ensuite éliminée par les reins. La métabolisation et l’élimination limitent l’action de la morphine à 2-3 heures.

Source text - English Most figures in this report include confidence intervals to visualize the sampling error associated with each finding. These confidence intervals represent the range of the estimate at a 95% level of confidence. This means that if the survey were re-run 20 times, the result obtained should fall within the range indicated by the confidence interval 19 of those 20 times. As a general rule of thumb, if the confidence intervals of two estimates overlap, then it is likely that there is no statistically significant difference between the estimates. If the confidence intervals do not overlap, then there is likely a significant difference between the estimates. However, there are exceptions to this general rule and readers are encouraged to rely on the report text and summary tables for definitive results on which comparisons or associations are significant and which not.
“Significant” here means that statistical tests show a contrast or association with a p-value of less than 0,5, a commonly-used threshold for a statistically significant result. Please note that the term significance is solely a statistical appraisal of a difference or relationship observed and does not necessarily mean that a finding or result is meaningful or notable from a programmatic perspective.
The analysis of the project impact was visualized with graphs like that in Figure 2, which shows the trend over time for both the target and comparison groups. Statistical analysis of the difference between these two trendlines was used to determine whether or not there was any statistically significant impact to report. The figure titles and text descriptions indicate whether there was any significant impact and whether it was positive or negative. For example, in Figure 2, although starting with similar opinions, the direct target group respondents became more positive about their capacity to negotiate a fair price for their produce, while the comparison group became somewhat less confident. There is a significant difference between these two trends, so we concluded that being in the direct target group was associated with a positive impact on this outcome.

Par exemple, pour la figure 2, même si au début les opinions étaient similaires, les répondants du groupe cible direct ont commencé à avoir une vision plus positive de leur capacité à négocier un prix équitable pour leurs récoltes alimentaires alors que le groupe de référence est devenu un peu moins confiant à cet égard. Il existe une différence significative entre ces deux tendances, nous en avons donc conclu que le fait d’appartenir au groupe cible direct était associé à un impact positif pour cet effet recherché.
