• User payroll number

• Email
• An existing user who this new user should be modeled after.

Service Center Ticket required:
It is necessary to open a service center ticket with request information to document the original request and your update in SAP for audit purposes.

The user request can be copied into the description or files can be attached using the attachments tab."

An extract found in another document It seems that "registro no centro de serviços/assistência" makes more sense in the following context:

• User group assignment review – Report 1

User has left the company:
Business-User must lock the account immediately. LBA’s must request the CSC to lock the account immediately. CSC and the Business User must notify IT Security for account deletion via Service Center ticket.
